Reject duplicate AccessToken names (#10994)
* make sure duplicate token names cannot be used

* add check to api routes too

* add @lunny s suggestion

* fix & don't forget User.ID

* AccessTokenByNameExists() return error too

* unique token for each test

* fix lint
